On a phone it's in the menu. Either way it's the one that says Schedule →.
Read the short page that comes up.
It's called “A Two-Minute Read, Then You're Set” and it covers the
handful of things people get wrong when booking. Worth the two minutes the first time.
When you're done, tap Continue To Scheduling → at the bottom of it.
Pick which office.
Sinking Spring or Muhlenberg.

First time, or it’s been over three years? Choose Chiropractic — New Patient Exam & Treatment. It’s a longer slot, so there’s time to work out what’s going on and not just treat it.
The other two are for once you’re established:
Full Office Visit — the adjustment plus therapies (Stim, Ultrasound, Cold Laser, Graston, Shockwave). This is what most of our patients book. Adj. Only — just the adjustment. Pick this only if you’re not having therapies. Most insurance covers the therapies; Medicare and Cigna are the exceptions.
Worth getting right the first time. Booking the
wrong one costs about 40 minutes of the doctor's day — time two other people could have
been seen in. And switching once you're here isn't guaranteed either way; it comes down to the
doctor's call and what's left on the schedule.

Cancelling or Moving an Appointment
You can do both yourself, any hour of the day — you don't have to wait for us to open.
Can't make it at all
Cancel it
Sign in, open your upcoming appointments, choose the visit, cancel. That's the whole thing.
Just need a different time
Move it instead
Same three taps, but pick a new slot rather than cancelling. Easier than starting over,
and you keep your place with us.
The 24-hour rule
Give us 24 hours and there's no charge at all — easy, no hard feelings.
Inside 24 hours it's nonrefundable, and that one doesn't bend.
But tell us what happened anyway. A work emergency, a sick kid, car trouble
— let us know and we'll put the real reason on your file. It matters more than you'd think.

Getting a Copy of Your Records
Your chart is yours. We just need it in writing so we know we're sending the right thing to the right person.
Tell us what you actually need.
The whole chart, or a date range? Just the X-rays or imaging report? A note for work, school or a lawyer?
Narrowing it down makes it far quicker.
Tell us where it's going.
To you, to another doctor, to an attorney or an insurer — and how you'd like it sent.
We send it.
How long, and what it costs. There’s no fee. Give us five business
days — and if your chart runs over 50 pages, please allow us a little longer, and
we’ll get it right to you.
Insurance Questions
The short answer: send us your card and we'll find out for you before you come in.
Take a photo of the front and back of your card.
Send it over, or bring it to your first visit.
If you send it ahead we can check your coverage before you're sitting in the chair.
We'll tell you what to expect.
What's covered, what your visit is likely to cost, and whether you've got a deductible to meet first.
Plans change constantly, even mid-year. Checking takes us a couple of minutes and saves a surprise later.
Hours and How to Find Us
Two offices, one team — same phone, same hours. Pick whichever is closer.
Near Adelphia Seafood, same side as the Wawa, a couple of houses down toward the
Rita's & McDonald's intersection. At the light just before Lidl make a right, then another
immediate right into the back alley where everyone parks. Look for the white gazebo —
any of those spots work. You can park out front on Penn Ave, but the big lot is round the back.
Muhlenberg — 3646 Pottsville Pike, Reading, PA 19605
On Route 61 next to 5th Street, down from the Outer Station and the Clover Farms
dairy plant, right before the bridge with the Wawa on the corner. We're the big blue barn
between the Dunkin' and the Wawa.
If something doesn't look right, ask — and if you need an itemized receipt for your HSA or FSA, this is where to get it.
If insurance is still working on it, we'll say so. A balance often just
means your plan hasn't finished paying yet — it isn't always money you owe.
THE SAVINGS PLAN · $25/MONTH
Save 25% On Everything.
For $25 a month, you get 25% off every service we offer — chiropractic, massage, stretch, all of it (massage and stretch are interchangeable). One discount at a time, though — the membership doesn’t combine with other specials. Just ask your therapist to sign you up while you’re in the office.

Booking Massage and Chiropractic the Same Day
Two separate bookings, and the gap between them matters more than people expect.
Book them as two appointments.
One for massage or stretch therapy, one for chiropractic.
Leave at least 20 minutes between them.
Thirty is safer. A lot of places sell an “hour massage” that's really about
fifty minutes once you're settled and wrapped up — ours is a true hour, hands on.
Here's the maths.
A 60-minute massage starting at 1:00 means booking chiropractic
after 2:20.
On a busy day the doctor can run five or ten minutes behind, which is why
thirty minutes is the safer gap.
Our honest recommendation: book this one at the front desk while you're
finishing up the appointment you're already in — or tell us and we'll set it up for you. Try it
yourself by all means, but of everything on this page, this is the one that trips people up.
Putting Booking on Your Phone Like an App
Worth doing once. After this it's an icon on your home screen — no hunting for the website.
Open smithcw.janeapp.com in your phone's browser.
iPhone: tap the Share button — the square with the arrow coming out of it —
then scroll down and tap Add to Home Screen.

Android: tap the three dots in the corner, then Add to Home screen.
Name it and you're done.
It'll sit with your other apps.
If Something Happened
A car accident·An injury at work
If You Were in a Car Accident
Come in and get looked at. The paperwork side is simpler than people expect:
you pay at the visit, and we give you everything you need to send to your auto insurance.
Book it as a new patient visit — even if you've been here before.
We mean that. An accident visit is more involved than a regular one, and the
new-patient slot is longer, so we actually have the time to look at everything properly.
Don't wait to sort the insurance out first either — getting examined early matters more,
both for how you feel and for the record.
Keep these handy for your reimbursement form.
Your insurance carrier · claim number · date of the accident · your adjuster's name, phone, email and fax.
Have an attorney? Let us know.
We give your records to you, so you know where they need to go and can hand them to your lawyer.
Then just come to your visits.
We'll give you the itemized receipt and the form to send to your adjuster. If they aren't ready at your visit, allow up to two weeks. We're a small office and pull paperwork together every two weeks.
Don't have the claim number yet? Come anyway. Bring it whenever it turns up.
Any auto policy, any state. Because you pay at the visit and send in the forms yourself, it doesn't matter where the accident happened or where your policy is written. We can help with every kind of claim now.
If You Got Hurt at Work
Work injuries run on their own track, and it's mostly about having the right claim
details from the start.
Report it to your employer first, if you haven't.
That's what opens the claim. Without it there's nothing to get reimbursed from.
Keep these handy for your reimbursement form.
Your workers' comp carrier · claim number · date you were hurt · employer's name · your adjuster's name, phone, email and fax.
Book it as a new patient visit — even if you've been here before.
A work injury needs the longer slot. Book the new-patient visit so there's
time to examine it properly and get the claim started right.
Tell us at booking that it's a work injury,
so we book the right visit and note it on your chart from day one.
The 90-day rule — check this before you book. In Pennsylvania, for the
first 90 days of treatment on an accepted work injury, you generally have to see a provider
from the list your employer posted. After 90 days you can see whoever you want, us included.
If you're still inside that window, check whether we're on your employer's list first. If you see someone who isn't on the list during those first 90 days, your carrier may not pay you back.
We don't bill workers' comp, and we don't check your claim for you. You pay at the visit, and we give you everything you need to send in for reimbursement.
For Attorneys, Adjusters & Other Offices
Requesting records from us
Requesting Records From Us
For an injury case, the easiest route is through your client. We give patients their own records, and they can pass them straight to you. Subpoenas and court orders still come here.
For attorneys’ offices, adjusters, and other clinics. You can send the whole request
from this page — the form below asks for everything a complete one needs, so it doesn't
come back to you twice.
Make sure the patient has authorized the release — unless you're subpoenaing.
Most of our patients have already signed an authorization on their intake form,
so often it's already covered. If it's a subpoena or a court order, send that instead and we'll
act on it. If you're not sure which you've got, ask us and we'll tell you.
Identify the patient properly.
Full legal name and date of birth. Names alone are not enough — we
have duplicates.
Give us a date range, not "everything."
For an accident or injury case that's usually the date of loss forward. An
open-ended request takes far longer and buries you in pages you don't want.
Say which documents you actually need.
Treatment notes, imaging reports, the itemized billing ledger, and a narrative
report are four different things. Ask for the ones you need.
Tell us where to send it and who to call.
A named contact and a direct number saves a round trip.
Rather send it another way?
Fax(610) 678-4747
— goes straight to the office, any hour.
Call(610) 678-8600
— Mon–Fri 9 AM–7 PM. Nobody sits on the phone; this line is for return calls.
Using AI to write the request? Paste this in.
Hand this to ChatGPT, Claude or whatever your office uses, answer what it asks,
and it will produce a request that has everything we need — first time, no back and forth.
You are drafting a medical records request to a chiropractic office on behalf of my firm.
Ask me for anything below that you don't already have, then write the request as a short,
professional letter. Do not invent any detail — if I don't give it to you, ask.
WHAT THE OFFICE REQUIRES:
1. Patient's full legal name AND date of birth (both — name alone is not enough).
2. A signed, current HIPAA authorization from the patient, attached.
3. A specific date range. For an injury case, state the date of loss and request records
from that date forward. Do not ask for "all records" without a range.
4. A specific list of what is needed. Pick from:
- treatment notes (SOAP notes) for the date range
- imaging reports (X-ray / MRI / CT findings)
- itemized billing ledger with CPT codes and charges
- a narrative report from the treating doctor
- the initial examination findings
5. If this is an auto or workers' compensation matter, include:
- insurance carrier name
- claim number
- adjuster name
- date of accident or injury
6. Our firm's name, the requesting contact's name, direct phone number, and the exact
address or secure email/fax where records should be sent.
7. Your deadline, if you have one, and whether the matter is in active litigation.
FORMAT:
Put the patient's name and date of birth at the very top. Keep it to one page.
List the requested documents as a numbered list. State plainly that the signed
HIPAA authorization is attached.
Turnaround and cost. Allow ten days for an attorney’s office or
adjuster. A patient’s own chart is five business days and carries no fee. An attorney’s
request does carry a copying fee, and we’ll give you the figure before anything is charged.
None of These
Just talk to us
If You'd Rather Just Talk to Someone
Completely fine — but here's the honest part first. There's nobody sitting by
the phone waiting to pick up. It's a small but busy office, and everyone is either with their
family or in a room with a patient. So leave a message, or text, and a real person comes back to you.
(610) 678-8600 — give us a call and
leave a message and we'll return it, or shoot us a text with what you're looking for. Either way,
tell us whether you want a call back or a text.
Tell us everything. Where you got stuck, what you're trying to do,
and anything else you can think of — the more you give us, the more we can have finished
before we even call you back.
Like this: “I'd like to book at Sinking Spring on Wednesday between
three and five with Dr. Smith.”
It's always quicker to do it yourself — but if you need something
moved, or you've genuinely got a question, just tell us the nature of it. We'll come back with the
answer, or set up a time to get on the phone properly.
